Tomando Control de su Diabetes

This is the Spanish version of the Diabetes Self-Management Program that was designed in Spanish for Spanish - speakers. People with type 2 diabetes attend the class in groups of 12-16. Classes are facilitated from a highly detailed manual by two trained Leaders, one or both of whom are peer leaders with diabetes themselves. Participants will make weekly action plans, share experiences, and help each other solve problems they encounter in creating and carrying out their self-management program. Physicians, diabetes educators, dietitians, and other health professionals both at Stanford and in the community, have reviewed all materials in the class.

About the class

Duration of a workshop:

  • Workshops meet weekly for six weeks for 2.5 hours with a built-in break
  • Workshops meet in person at convenient community and clinical settings
    OR virtually (technical assistance is provided)

Topics covered during workshop sessions:

  • Nutrition, healthy eating and menu planning
  • Physical activity and exercise
  • Medication management, monitoring and foot care
  • Working more effectively with health care providers
  • Problem solving, decision making, goal setting and many more

Who should take the class

  • Adults diagnosed with pre-diabetes or diabetes and their caregivers
  • Adults whose health problems have begun to interfere with daily activities
